This happened to a Chinook while out in Kuwait. The details of how the accident happened I do not know. I was just one of the three to help it land.
At the end of the video the Safety Officer is asking for a copy of the video. He states, "I want a copy of that alright... Seriously, it'd be great training."

TF Centaur CH-47D at FOB Naray (Kunar Province) lifting one set of a few sling loads bound for 3-71 CAV soldiers at Camp Keating and Gowardesh in 2006. The AH-64A belonged to 7-6 CAV. Flights into this territory always required a gunship escort as they were targeted by Taliban and HiG forces (among others).
